How long does creatine take to work? The evidence-based timeline

Creatine effects do not all arrive at once: water enters the muscle within the first week, muscle saturation takes 6 days with a loading phase or 28 days without one, and performance gains follow saturation rather than preceding it. Roughly 28 per cent of people do not respond at all, because their muscle stores were already close to the ceiling before they started. This article sets out the timeline from the biopsy and isotope-dilution studies, not from marketing claims.

The four numbers that matter

  • Week 1: +1.37 litres of total body water, before any muscle gain.
  • Saturation: 6 days at 20 g per day, or 28 days at 3 g per day. Same endpoint.
  • Non-responders: 28 per cent of subjects in muscle biopsy studies.
  • On stopping: return to baseline within 30 days, with no rebound below it.

Week one: what actually moves the scale

Water, not muscle. This is the first measurable effect and it explains essentially all of the initial weight gain.

The clearest measurement comes from Powers and colleagues, published in the Journal of Athletic Training in 2003. Thirty-two resistance-trained participants, sixteen men and sixteen women, were randomised double-blind to creatine or placebo. Total body water was quantified by deuterium oxide dilution and muscle creatine by biopsy of the vastus lateralis, rather than estimated.

The first-week findings were unambiguous:

  • Total body water rose by 1.37 litres in the creatine group.
  • Body mass rose by 0.75 kg, which did not reach the study’s significance threshold of 0.88 kg.
  • One participant, whose weight had been stable for a year, gained 4.8 kg in a week, of which 90 per cent was explained by the increase in body water alone.

The practical conclusion follows directly. Two kilograms gained in five days is not muscle, and it is not a reason for concern. The distribution of fluid between the intracellular and extracellular compartments also stayed normal, which undermines the long-standing theory that creatine causes cramping through fluid shifts.

Six days or twenty-eight: two routes to the same plateau

Both routes reach the same endpoint. The difference is speed, not outcome.

The reference study remains Hultman and colleagues, published in the Journal of Applied Physiology in 1996, on 31 male subjects receiving varying doses over varying durations.

In other words, taking 3 g per day with no loading phase reaches exactly the same muscle level, twenty-two days later. Choose the loading phase only when there is a deadline: a competition in ten days justifies it, general year-round training does not.

One practical point when comparing products: choose one that states the creatine monohydrate dose per serving in clear figures rather than hiding it inside a proprietary blend. Without a stated dose per scoop it is impossible to run either protocol accurately.

When do performance effects appear?

After saturation, never before. The muscle has to fill its reservoir before the additional energy substrate becomes available.

The International Society of Sports Nutrition position stand, published by Kreider and colleagues in 2017, places the total creatine and phosphocreatine pool at around 120 mmol per kilogram of dry muscle, against an upper storage limit of about 160 mmol/kg. Supplementation fills that gap.

In practice, allow one week with loading and three to four weeks without. The effects apply to short, intense efforts of roughly 5 to 15 seconds, where phosphocreatine is the limiting fuel. Someone stuck at 8 repetitions with 100 kg on the bench press will manage 9 after three weeks, not 11.

That nuance matters, because it avoids two symmetrical errors: expecting a dramatic transformation and being disappointed, or measuring nothing at all and never establishing whether the product works for you.

Why 28 per cent of people feel nothing

Because their reservoir was already nearly full. This is not an estimate; it comes from the same biopsy study.

Powers and colleagues reported that 4 of the 14 subjects analysed, or 28 per cent, did not respond to supplementation. This aligns with earlier work by Greenhaff, which placed the non-responder proportion at 20 to 30 per cent of the population.

The explanatory factor is identified and quantified. Baseline muscle concentration ranged from 105.9 to 150.8 mmol/kg of dry muscle across participants, with a mean of 125.6. The authors observed a strong negative correlation between that starting level and the gain achieved (r = -0.75, p < 0.01):

  • Subjects starting below 120 mmol/kg gained on average 33.4 mmol/kg, or +23.1 per cent.
  • Subjects starting above 120 mmol/kg gained only 16.6 mmol/kg, or +11.2 per cent.

The gain is therefore roughly twice as large in those who started low. Since dietary creatine comes almost exclusively from meat and fish, a heavy red meat eater arrives naturally close to the ceiling and will notice little. A vegetarian, whose stores sit between 90 and 110 mmol/kg according to the ISSN, is in the opposite position and is the most likely to respond strongly.

One detail of the study is worth noting: the non-responders were spread across the whole weight range, from 57 to 86 kg. It is therefore not a question of the dose being too small for heavier individuals.

What happens when you stop?

A gradual decline, with no rebound effect and no sudden loss. Hultman’s study measured the return to baseline precisely: without a maintenance dose, total muscle concentration declines gradually and, 30 days after cessation, is indistinguishable from the pre-supplementation value. There is neither dependence nor a drop below the starting level.

That timeline has a useful practical consequence. A one-week break during a holiday does not reset anything, because the decline is slow and the level stays high. Stopping for two months, on the other hand, means starting again from the beginning, with another one to four weeks before saturation returns.

How to check objectively whether it is working

Look at performance, not the scale. Body weight tells you nothing useful in the first three weeks, because most of what it is measuring is water.

  1. Record loads and repetitions on two or three reference exercises for the four weeks before supplementation and the four weeks after.
  2. Compare at equal training volume. Do not change the programme at the same time as adding the supplement.
  3. Look at longer sets of intense effort, where phosphocreatine genuinely contributes, rather than at a one-repetition maximum.

Regulatory position and safety

In the European Union, two health claims relating to creatine are authorised under Regulation (EC) No 1924/2006 and its implementing lists. The primary one states that creatine increases physical performance in successive bursts of short-term, high-intensity exercise, with a condition of use of 3 g of creatine per day. A second, narrower claim covers the contribution of daily creatine intake alongside resistance training to muscle strength in adults over 55. Any other claim made on a label or in a facility is not authorised.

On safety, the ISSN position stand reports that supplementation of up to 30 g per day for five years has been shown to be safe in healthy subjects. National food safety agencies across Europe nonetheless advise against muscle-building supplements for people with cardiovascular risk factors, cardiac disease, impaired renal or hepatic function or neuropsychiatric conditions, and for children and adolescents. Where a facility stocks or recommends supplements, specify products carrying an independent batch-testing certification, which is the practical safeguard against contamination with substances prohibited in sport.

How long does creatine take to work?

Is a loading phase necessary?

No. Loading and steady low-dose intake reach the same muscle creatine plateau; loading simply arrives twenty-two days earlier. It is worth doing before a competition and unnecessary for general training, where it mainly increases the initial water retention and the gastrointestinal discomfort some people experience.

Why does creatine make you gain weight in the first week?

Because creatine is osmotically active and draws water into the muscle cell. Total body water rises by around 1.37 litres in the first week, which accounts for essentially all of the early weight change. It is not fat and it is not yet muscle.

Why do some people not respond to creatine?

Because their baseline muscle creatine is already near the storage ceiling, usually as a result of a diet high in meat and fish. Subjects starting below 120 mmol/kg of dry muscle gained roughly twice as much as those starting above it. Vegetarians, with the lowest baseline stores, respond most strongly. Changing brand does not change this.

What happens when you stop taking creatine?

Muscle concentration declines gradually and returns to baseline about 30 days after the last dose. There is no rebound below the starting level and no dependence. A short break of a week has no meaningful effect; a break of two months means starting the saturation process again.
